derc adult registration
- Please use the form below to register ADULT PARTICIPANTS for the Diocesan Eucharistic Revival Congress. Adults may attend more than one session. Each session is $20.00.
  
  Youth Ministers and Parents/Guardians who are planning to accompany their youth at a Youth Congress session should also use this form to register. All DERC Youth (11-17 years old) registrations should be submitted through the Youth Form (link to the right).
  
  Dates of Congress Sessions for Adults:
  
  Spanish Adult Track: Saturday, September 9, 2023, 7:00am - 3:00pm
  
  English Adult Track: Saturday, September 16, 2023, 7:00am - 3:00pm
  
  Youth Track participation also available for Youth Ministers and for parents/guardians accompanying their children
  
  All registrants will be required to submit a signed photo permission and release document.

- Todos los participantes de DERC (Congreso Diocesano de Avivamiento Eucarístico, por sus siglas en inglés)- deberán firmar un FORMULARIO DE CONSENTIMIENTO GENERAL + AUTORIZACIÓN PARA FOTOGRAFIAR, GRABAR/FILMAR Y DISTRIBUIR PARA TODOS LOS USOS DIOCESANOS, que esté archivado por el Comité Diocesano del Congreso de Avivamiento Eucarístico.
- Por favor utilice el siguiente formulario para inscribir PARTICIPANTES ADULTOS en el Congreso Diocesano de Avivamiento Eucarístico. Los adultos pueden asistir a más de una sesión. Cada sesión cuesta $20.00.
  
  Los Ministros de la Juventud y los padres/tutores/acudientes que planean acompañar a sus jóvenes en una sesión del Congreso de la Juventud también deben usar este formulario para registrarse.
